One of the causes of this is when people focus on Scrum as a 'methodology' instead of the underlying purpose. It is easy to 'implement' Scrum by changing a few job titles, adding 'Scrum meetings' to an already overcrowded schedule, and hope for the best.
While this mechanical view is understandable, it misses the entire purpose of the framework. The Scrum framework exists to reduce the risk of wrong assumptions about what stakeholders need. This includes what stakeholders assume their needs are in the first place. Scrum is all about addressing those needs in small steps, starting with the most important ones, and using each step to test both the need and the proposed solution. Obviously, you cannot do this without close collaboration with stakeholders (users and customers).
